FEES AND INSURANCE

At this time, I am in-network with Pacific Source and First Choice Health Network. This means that I am out-of-network with most other insurance companies and can send out-of-network claims on your behalf. However, I CANNOT send claims to Medicare or Medicaid since I have opted out of that service. If you have Medicare or Medicaid, please let me know prior to starting services so that we can talk through your options.

Please note that the client is responsible for any fees (or portion of the fees) insurance does not cover. If you pay out of pocket, I can provide you with a superbill so that you can turn that into your insurance company for reimbursement.

Do I accept Sliding Scale? Yes, I have a handful of sliding scale spots available. They are time-limited and dependent on your financial situation.

Did you find me through Lyra Health? These rates do not apply to you and will vary depending on your company benefits. Please contact your benefits coordinator through your work for more information.
